COI  and CFS

Postural orthostatic tachycardia syndrome is an under-recognized condition in CFS, hoad.etal.08.txt, hoad.etal08.txt, hoad.etal08.pdf.  Higher heart rate at night in CFS, boneva.etal07.txt.

A review on orthostatic intolerance in CFS/ME, spence.stewart04.pdf. Orthostatic instability was similar in persons with CFS and nonfatigued controls, jones.etal06.txt, jones.etal06.pdf, a result questioned, rowe.lucas07.txt.

On CFS exercise intolerance,  Behan et al., 2001.  Dr. Bell, an expert in this area, recommends this procedure,  Bell, procedure, 2000. 

The hemodynamic instability in CFS, Naschitz et al., 2001,   Naschitz et al., 2002,  including fractal analysis,  Naschitz et al., 2003,  Naschitz et al., 2003 (pdf).  Dysautonomia is caused by shortened QT interval, naschitz.etal.06.txt,  naschitz.etal.06.pdf.  Another group found autonomic instability in CFS, but atributed this to deconditioning,  winkler.etal04.txt, winkler.etal04.pdf.

Blood flow instability lowers oxygen delivery to muscles, but this does not influence muscle metabolism in CFS, mccully.etal03.txt, mccully.etal03.pdf.
 

Hypotension in early adulthood may predict idiopathic CF later in life, lucas.etal04.txt.

 

Autonomic Nervous System in FM

Cutaneous microvascular circulation was normal in FM,  Al-Allaf et al., 2001.  However, an interesting finding was made by Swiss researchers, ‘Microcirculation abnormalities in patients with fibromyalgia  measured by capillary microscopy and laser fluxmetry’, morf.etal05.txt.

COI in FM versus CFS

One study measured OI in FM and CFS and found a difference,  Naschitz et al., 2001. According to these researchers there is no specific cardiovascular reactivity pattern in FM, naschitz.etal05.txt. Patients with Lupus have COI (also named NMH), but this symptom was not a significant determinant for bad health if patients simultaneusly had FM, tang.etal04.txt.



The Tilt Table Test

A head-up-tilt-test (CHUTT) has proven reliable with CFS patients,  Naschitz et al., 2000,  and an "instability score" for CFS problems found with this test,  Naschitz et al., 2002, and  Naschitz et al., 2003.  Patterns of hypocapnia on tilt in patients with FM, CFS, nonspecific dizziness, and neurally mediated syncope, naschitz.etal06.txt,  naschitz.etal06.pdf. In a UK study, vasovagal syncobe (fainting) were often associated with CFS symptoms,  letter, Kenny & Graham, 2001.  In a recent study, this test was found valuable, even diagnostic,  Timmers, 2002.  Cerebral blood flow in CFS was not different from normal during the test,  Razumovsky et al., 2003.  A New Jersey study elaborated the heart rate variability found in the head up tilt (HUT) test,  Yamamoto et al., 2003.

 

COI in Adolescents and Children

Low erythrocyte volume combined with COI,  Streeten et al., 2000.

A very thorough study demonstrated attenuated vasovagal reflexes behind POTS and CFS to be the same in adolescents,  Stewart, 2000.  Dr. Julian Stewart also wrote this review on OI in CFS,  Stewart, 2001,   Stewart, 2001 (pdf).

A study of cerebral functioning concluded that fatigue is linked to orthostatic intolerance,  Tanaka et al., 2002,  a matter discussed in the journal,  editorial, Rowe, 2002,   editorial, Rowe, 2002 (full paper).

The Cause of COI in CFS

There is evidence for increased cholinergic sensitivity in CFS,  Spence et al., 2000.  An interesting study on a possible defect in venous innervation as the cause of CFS-COI,  Streeten, 2001.   Newer studies found more substantial evidence for the cholinergic thesis, kawamura.etal03.pdf, khan.etal.03.txt,  khan.etal03.txt,  khan.etal03.pdf, including the discovery of autoantibodies against cholinergic receptors, tanaka.etal03.txt.  Further evidence for a difference between CFS and GWS in relation to cholinergic response, khan.etal04.txt, khan.etal04.pdf and spence.etal04.txt, spence.etal04.pdf.
 

The New Jersey team has suggested that a subtle underlying disturbance in autonomic function may occur in patients with CFS, yoshiuchi.etal04.txt, yoshiuchi.etal04.pdf. 

A Norwegian group studied sympathetic predominance of cardiovascular regulation, wyller.etal07.txt, wyller.etal07.pdf and abnormal thermoregulatory responses in adolescents with CFS: Relation to clinical symptoms, wyller.etal.07.txt, wyller.etal.07.pdf. Also during orthostatic stress and isometric exercise, wyller.et.al07.txt, wyller.et.al07.pdf.

The vascular instability in conditions like CFS is posibly caused by autoantibodies (vasoactive neuropeptide autoimmunity), staines06.txt, staines06.pdf.

Ortostatic hypocapnia disturbes test results in a subgroup, natelson.etal.07.txt, natelson.etal07.txt.

 

Baroceptors in CFS

Baroceptor studies have revealed interesting results in CFS, peckerman.etal03.txt, peckerman.etal03.pdf and peckerman.etal.03.pdf.
